Diabetes and Metabolic Syndrome Counseling

Treatment Guidelines:
Collaborate with patient’s Primary Care Physician to obtain labs and assess for potential factors which could be affecting weight or behaviors around food
  • Diabetes:
    • If patient is insulin-dependent, dietitian to collaborate with patient’s Primary Care Physician around this
    • Assess labs specific to diabetic needs
    • Review patient’s behaviors around food/insulin and blood sugar records if possible
    • Educate patient on health complications of diabetic non-compliance if needed
    • Provide nutritional and exercise recommendations with a focus on stabilization of blood sugar
    • Assist patient in changing dietary and exercise habits to meet individual goals
  • Metabolic syndrome:
    • Obtain and analyze labs specific to metabolic syndrome
    • Assess whether or not he or she is at risk for gout
      • If so: dietitian to provide medical nutrition therapy around gout management
    • Monitor patient’s blood pressure to assess for hypertension
    • Review current food intake and exercise patterns
    • Provide patient nutritional and exercise recommendations with a focus on normalizing weight, vitals and labs over time

 

Weight monitoring determined on individual basis vitals and labs over time
